Please complete all required fields. Once you have completed the request and your UCI DCE has been located, you will receive a Change of Email form via Docusign to your current email address. Once you complete the Docusign form, the Records office will change your email address and you will receive a confirmation notification. Requests received after business hours will be processed the next business day.
